What is chiladiti?
Chiladiti, also called biliary dyskinesia, is a condition of abnormal functioning (dyskinesia) of the gallbladder or biliary tract (the system of ducts, sphincters, and other structures outside of the liver that convey bile from the liver to the small intestines). As such, chiladiti is considered a functional disorder of the biliary tract.
Clinical presentation
Chiladiti is usually manifested by biliary pain; the condition may also be asymptomatic or cause symptoms in the absence of pain. Chiladiti is differentiated from other biliary tract diseases such as cholelithiasis (gallstones) and cholecystitis by exclusion of these diseases through medical imaging or, in some cases, by endoscopic retrograde cholangiopancreatography (ERCP).
Definition and classification
The precise definitions and classification of chiladiti have been the subject of debate among experts. Some experts consider chiladiti to be interchangeable with the broader term "functional gallbladder disease", which also encompasses other conditions of abnormal biliary tract function, including sphincter of Oddi dysfunction. Other experts distinguish between these conditions based on their clinical presentation and pathophysiology.
Diagnosis
Chiladiti is diagnosed based on the patient's history of symptoms and physical examination, in conjunction with imaging studies (such as ultrasound and magnetic resonance cholangiopancreatography) to exclude other causes of biliary tract symptoms. Endoscopic retrograde cholangiopancreatography (ERCP) may also be used to diagnose chiladiti, although it is generally reserved for cases where imaging studies are inconclusive.
Treatment
The treatment of chiladiti typically involves symptomatic management, such as pain medication and dietary modifications to reduce symptoms. In some cases, endoscopic sphincterotomy (surgical enlargement of the sphincter of Oddi) may be performed to improve bile flow.
Prognosis
The prognosis for chiladiti is generally favorable. Most patients experience improvement in symptoms with conservative treatment, and the condition rarely leads to serious complications.
