How Orladeyo Works for Hereditary Angioedema

Orladeyo (berotralstat) works by selectively inhibiting the plasma kallikrein enzyme. Plasma kallikrein is a serine protease that plays a central role in the kallikrein-kinin system, which is involved in various physiological processes, including blood pressure regulation, vasodilation, and inflammation.

In hereditary angioedema (HAE), Orladeyo helps prevent attacks by targeting the underlying mechanism of the condition. Here's how Orladeyo works:

Inhibition of Plasma Kallikrein: Orladeyo binds to and inhibits plasma kallikrein, preventing its activation. By doing so, it blocks the subsequent steps in the kallikrein-kinin system, including the production of bradykinin.

Bradykinin Inhibition: Bradykinin is a potent vasoactive peptide that increases vascular permeability, leading to the characteristic swelling and tissue edema seen in HAE attacks. Orladeyo's inhibition of plasma kallikrein ultimately reduces bradykinin levels, preventing the development of angioedema symptoms.
