1
Complete the MedicAlert enrollment form (see Resources). Expect to provide basic contact information, such as your name and address, emergency contacts and information about your medical conditions. The emergency contacts listed here will be called when MedicAlert is informed of an emergency.

2
Choose one of the bracelets shown on the enrollment form and enter payment information for the second year of membership. Expect to be charged a fee of $20 per year if you are under 18 years old or $35 if you are an adult on the renewal date. Alternatively, you may check the box to instruct MedicAlert not to charge you automatically. If payment is not made on the renewal date, your membership will be canceled.

3
Write a brief statement requesting sponsorship due to financial hardship, and provide proof of hardship. You may provide proof of government assistance, a recent income tax return, a letter attesting to your inability to pay from a doctor, nurse or case worker, or proof of Social Security disability insurance.

Mail your completed form and supporting documentation to MedicAlert at 2323 Colorado Ave., Turlock, CA 95382. Wait for a response to see if you are granted a free 1-year membership and bracelet.
