How Do I Make an HFA Inhaler More Effective?
Metered-dose inhalers (MDIs) are used in the treatment of asthma, allergies and other chronic and acute respiratory conditions. MDIs deliver drug product to the respiratory tract in aerosol form, via a propellant.In 2005, the U.S. Food and Drug Administration (FDA) required inhaler manufacturers to cease use of chlorofluorocarbon (CFC) propellants, due to potentially harmful effects on the environment. The CFC propellants in inhalers have since been replaced by hydrofluoroalkane (HFA).
If you have used a CFC inhaler in the past, you may feel HFA inhalers do not provide the same level of benefit. However, the overall form and function of MDIs has not really changed. Proper storage, handling and inhalation technique and use of an extension chamber--or spacer--may help improve your experience with an HFA inhaler.

Proper MDI Storage and Handling

Store all inhalers in a cool, dry place away from direct sunlight to avoid any deterioration of drug or propellant.

Keep inhaler mouthpiece clean and free of debris or obstruction.

Know the number of puffs or actuations your inhaler is expected to deliver. This information may be found on the product package or supplied by a physician or pharmacist. Be mindful of the number of actuations remaining in your inhaler.
Proper MDI Technique

Sit or stand, so the torso is erect. Tilt chin upward slightly to open airway fully.

Remove the mouthpiece cover and shake inhaler thoroughly to distribute drug evenly within the device.
If you have not used the inhaler in several days, a priming dose may be required to ensure proper function. Point MDI away from face and depress canister once or twice into the air.

Exhale to clear lungs. Place MDI mouthpiece in mouth, seal lips around it and, while inhaling, depress canister.

Hold inhaled breath for 10 to 15 seconds if possible. Exhale.
If your dosage requires greater than one actuation, wait 20 to 30 seconds before repeating process.
Using an MDI and Spacer

Using an MDI properly does require a certain level of coordination. If you cannot manage this, an extension chamber, called a spacer, may be a useful option. Spacers hold the dose a few extra seconds, allowing you to breathe in at your own pace.
Remove MDI mouthpiece cover and shake inhaler well. Attach the spacer to the MDI mouthpiece as specified by the manufacturer.

Exhale to clear lungs. Place spacer mouthpiece in mouth, seal lips around it and depress canister, releasing drug into the spacer.

Inhale slowly, holding breath for 10 to 15 seconds, if able. Exhale. Do this twice for each puff delivered into the spacer.

If your dosage requires greater than one puff, wait 20 to 30 seconds, then repeat process.
