Fill out the application carefully, and sign and date the last page. Send the application to the address listed in step 1 along with proof of your income for the past 30 days and a signed prescription for the asthma medication from your doctor. If your asthma medicine is needed right away, have your doctor call Bridges to Access and request “same-day access.“

Wait 7 to 10 days for your asthma medicine to arrive in the mail. Asthma medicine will be sent to the address on the initial application. Refills must be requested at least 3 weeks before your asthma medicine runs out. If you were enrolled by your doctor for “same-day access” you will receive a voucher from your doctor that you will need to bring to a local pharmacy to receive your initial asthma medicine.
