In order to apply you must complete an application, which is available online, including the financial section verifying your income. You must also attach a prescription from your prescription for a 90-day supply.
Once you are approved your medication will be sent out in a 90-day supply. The medication can be sent either to the individual or the medical provider. You can continue to get your prescription for 12 months as which time you must reapply for the program.
