A small Duke University study conducted by Dr. James D. Lane concluded that caffeine may have a harmful effect on glucose levels. Other studies are inconclusive about the effects of caffeine and more research needs to be done.-

Some studies show that caffeine, particularly regular coffee, decreases insulin sensitivity and increases glucose levels in diabetics when consumed with carbohydrates.

No study has been large enough to be conclusive so people with fluctuating glucose levels should consult regularly with their doctors.
Prevention/Solution

Since experts can't agree, people with varied glucose levels should consume caffeine in moderation and consider switching to decaf ,which doesn't seem to have the same effect as caffeinated coffee.
