Forms to File for Disability

The forms to fill out for a Social Security disability claim are fairly straightforward. You must first provide documentation of citizenship which establishes your right to make a claim. Other forms include the basic application form, a form that describes your medical condition and a consent to release form. Most of the process can be completed online, though you can also call your local Social Security office for help or to file the application over the phone.
  1. Initial Documents

    • Before filling out any of the forms required by the disability process, gather all supporting documentation that you will need. The first necessary document is proof of citizenship in the form of either a birth certificate or naturalization papers to prove your initial eligibility. You also should have the names and addresses of doctors who are familiar with your medical condition, check stubs, tax returns and W-2 or 1099 forms to demonstrate your recent work history.

    Application

    • The application itself asks for your name, gender, social security number, marital status, children, sources of income and employment history. This form also asks you to list the details of your disabling medical condition or injury and how it prevents you from working. This is the foundational document for your claim. It will be sent to a claims determination case worker who will put all forms and supporting information into a file for review.

    Medical Condition

    • Form SSA-3368 is where you go into greater detail concerning your medical condition. Attach supporting statements by doctors familiar with your condition or illness. The Social Security Administration also wants copies of your historical medical records as well as current treatments and prescription drugs. The more information you can provide on this form, the better it is for your case. What is included here is used to determine if you actually have a disabling condition which falls within their definition.

    Information Disclosure

    • The other supplemental form to the main application is called Form SSA-827. Completion of this form authorizes Social Security to contact all the doctors, hospitals, clinics, labs, counselors and persons familiar with your condition for the release of official medical records for review. It is critical that you follow up to make sure the requested records are sent because your disability claim is evaluated primarily on the strength or absence of this documentation.
