Navigate to the application page by clicking on the link in Step 2 "Online Disability Benefit Application." Fill out the application with all requested information. You may need to provide personal banking information, such as account and routing numbers if you want your benefits deposited directly to your bank account.

Navigate to the "Adult Disability Report" by clicking on the highlighted link in Step 3. Fill out all requested information regarding your medical and work history to determine eligibility. Print off the finished forms and sign them. Mail the forms to your local Social Security Administration Office.
