Apply for Social Security disability insurance as soon as you are diagnosed with depression. You will be asked to provide a detailed explanation of your condition and how it affects your ability to work. Along with the application, the disability report is required. The report can be found at socialsecurity.gov/disabilityreport.

Gather all of the necessary documents required to be submitted with the application and disability report. You will be asked to provide details such as your Social Security number, birth certificate, current doctor and facility information, current medication names, medical records, most recent employment date and a copy of your most recent W-2.

Mail your application to the address provided or contact the Social Security office of Massachusetts at 1-800-772-1213 for additional processing information.
