  Download or request the application for disability. Obtain disability applications for the State of Ohio online from the Ohio Department of Job and Family Services website. Make a request in person at one of the offices, if you prefer. Find the list of disabilities that qualify for the application on the website as well.

  Obtain the necessary information to complete the application. Fill out page one and submit to the Ohio Department of Job and Family Services before completing any of the other pages. Page one includes household address, your name, and your signature.
