How do I File the Form for a Disability Permit in Florida?

If you are disabled and live in Florida, you may be eligible for a disability parking permit. There are two kinds of disability permits--temporary and permanent; temporary permits are issued to Floridians with a disability that is expected to last no more than six months, while permanent permits are issued to those who are either legally blind or cannot walk for more than 200 feet without resting due to a cardiac, arthritic, neurological, orthopedic or pulmonary medical condition. To find out if you are eligible for a disability permit, submit an application form to Florida's Department of Highway Safety and Motor Vehicles.

      Download and print an application for a Disabled Person Parking Permit from the official FLHSMV website or call the Customer Service Center at 850-617-2000 and request that an application be sent to you.

      Complete the top section of the application form. You will need to provide some information about yourself such as your name, address, birth date and your Florida driver's license or state-issued I.D. card number. Sign and date the application.

      Bring the application to your healthcare provider and have her complete the section, "Physician/Certifying Practitioner's Statement of Certification." Your physician must provide her license number and the state in which the license was issued.

      Make a photocopy of your Florida driver's license or I.D. Card.

      Prepare the appropriate fee, if applicable. Temporary parking permits cost $15 while permanent disability permits are free of charge.

      Mail or bring your completed application, photocopy of your identification and fee (if applicable) to your local county tax collector's office. Use the directory under Resources to locate the county tax collector's office nearest to you.
