How to Apply for Temporary Disability in Alabama
Things You'll Need
- Military discharge information (if applicable)
- W-2 form
- Social Security number
- Bank account and routing numbers (for direct deposit)
- All applicable hospital names, addresses and treatment dates
- Names of all applicable prescription medicines
- Names, location and dates of medical tests
- All jobs from the last five years
- Information on any other insurance or worker's compensation claims you have filed

Visit the Social Security Administration's online application page for disability benefits.

Start the application by answering two questions about yourself or the person you are apply for. Make sure you read the Privacy Act Statement and check the box stating that you have read it. You will not be able to continue to the application if you do not read the statement and check the box.

Complete the application. It will take you about 10 to 30 minutes to complete your application. Make sure you save your application confirmation number. This number allows you to check on the status of your application.

Complete the Adult Disability Report online. The Adult Disability Report can take up to 90 minutes to complete, depending on how many questions apply to you.

Download, print out and fill out the Authorization to Disclose Information to the Social Security Administration form. Mail the form to the closest Social Security office.

Check the status of your application by visiting the webpage for application statuses. You will need your Social Security number and confirmation number.
