How to Apply for Disability Forms

If you are disabled and you expect the condition to continue for at least a year, you may qualify for Social Security Disability. The earlier you begin the application process the better, because there may be a lengthy interval between the application filing and approval from the government.

      Social Security has a strict definition of disability, so make sure you're eligible. You qualify if your medical condition does not allow you to perform any substantial work, or if you expect your medical condition to last at least a year, or even result in death.

      Decide the appropriate disability benefit program. There are two programs: SSDI (Social Security Disability Insurance) for insured workers and SSI (Supplemental Security Income) for those with little or no income.

      Set up an appointment with the Social Security office. It can be via phone or in person. If the meeting is in person, it will help the case if you have other difficulties related to walking, listening or writing. The claims representative will record these observations. If the interview is over the phone, a representative from the Immediate Claims Taking Unit will take your claim. The toll-free number is 800-772-1213.       Fill out the medical job worksheet, disability report and other paperwork in the packet before meeting the claims representative. The representative will help fill out the form that will be sent to the state agency responsible for determining benefits. Usually, it will take between three and five months for your claim to be processed.
