If a patient experiences an adverse reaction to an injection, the medical assistant should take the following steps:

1. Immediately stop the injection and remove the needle from the patient's skin.

2. Assess the patient's condition. Check the patient's vital signs, including blood pressure, heart rate, and respiratory rate. Observe the patient for any signs of anaphylaxis, such as difficulty breathing, swelling of the face or throat, or hives.

3. Administer first aid. If the patient is experiencing anaphylaxis, administer epinephrine (EpiPen) according to the physician's orders. If the patient is not experiencing anaphylaxis, provide supportive care, such as oxygen therapy and intravenous fluids.

4. Notify the physician. The physician should be notified of the adverse reaction as soon as possible. The physician will determine the next course of treatment and provide further instructions.

5. Document the incident. The medical assistant should document the adverse reaction in the patient's medical record. The documentation should include the date and time of the reaction, the type of injection that was administered, the patient's symptoms, and the treatment that was provided.
