Patient Assistance for Lantus
Lantus is a long-acting form of insulin which only needs to be injected once a day. Sanofi-Aventis, the company that makes Lantus, offers a patient assistance program for patients who cannot afford to pay for the medication they need.-
Eligibility

Patients must have limited income and limited financial resources to qualify. They must not have health insurance. Patients must live in the U.S. Patients can use the Qualification Tool on the Sanofi-Aventis website to see if they qualify (see Resources).
Application Process

Patients or their physicians must complete an application. Their physicians must sign the Licensed Practitioner Section of the application. Patients must submit proof of income, such as paycheck stubs or income tax returns. Sanofi-Aventis reviews applications within three business days. Patients must reapply every year.
