Call the Social Security Administration to make an appointment to apply for disability at 800-772-1213. Or, begin an application online (see Resources).

Fill out the application completely. Do not leave any parts blank as the Social Security Administration may have trouble processing your application for scoliosis disability. If you need help filling out the application, ask for help at the local Social Security Administration office and staff will assist you.

Take the completed application to the local Social Security Administration office or submit an application online.

Provide information about income and resources when requested, such as paycheck stubs, a court order for child support or alimony and bank statements.

Inform you medical doctor of the application for scoliosis disability. The Social Security Administration contacts the doctor for information about your condition. Talk to your doctor and remind him how the scoliosis prevents you from working and ask him to document that information in your medical chart. Ask him to provide any requested information to the Social Security Administration promptly so as not to delay the processing of your application.

Forward any medical records copies to the Social Security Administration. They request information from the health care professionals that provide you medical treatment most recently but do consider all information sent to them. Sending them all copies of records allows them to fully understand your condition.
