The lingual groove of maxillary molars is an important anatomical landmark for endodontists and general dentists alike, as it provides essential information about the root canal anatomy of the teeth:

Distinguishing Between Roots: The presence or absence of the lingual pit can help in differentiating between maxillary first molars and second molars. In the majority of cases, maxillary first molars have a distinct lingual pit, while maxillary second molars usually do not. This distinction is useful when attempting to locate the mesiolingual and distolingual canals during root canal treatment.

Anatomic Anomaly: The lingual groove is an anatomic depression or concavity on the lingual surface of the tooth that corresponds to the developmental fusion of the protocone and hypocone cusps. This anomaly often leads to the formation of additional root canals, particularly the mesiolingual canal.
