How do you get a sanitary permit in the Philippines?

To obtain a Sanitary Permit in the Philippines, you must follow these steps:

1. Prepare the Necessary Documents:

a. DTI Business Registration Certificate (Original and photocopy)

b. Mayor’s Permit (Original and photocopy)

c. Zoning Certificate/locational Clearance from the local City/Municipal Planning and Development Office (Original and photocopy)

d. Lease Contract (Original and photocopy)

e. Food Handler's Certificate of the proprietor and 2 key personnel (Original and photocopy)

f. Medical Certificates of the proprietor and 2 key personnel (Original and photocopy)

g. Sanitary Permit Application form provided by the local government (Filled out properly)

h. Business TIN Number

i. Health Card from DOH (Original and photocopy)

j. Health Certificate of Business Establishment from DOH (Original and photocopy)

k. Certification of Compliance with Minimum Health Standards from DOH (Original and photocopy)

l. Site Development Plans (2 sets)

m. Vicinity Map (2 copies)

n. 3x4 picture of food handler (2 pcs.)

2. Visit the Local Health Office:

a. Go to the local Health Office in your area.

b. Submit the completed Sanitary Permit Application form and all the required documents to the Health Officer or designated personnel.

3. Health Inspection of the Establishment:

a. After reviewing your application, the Health Officer will conduct an inspection of your establishment to check for compliance with health standards and regulations.

4. Issuance of Sanitary Permit:
