Numbers to Call

Call the Appointment and Information Center at 734-763-6933 or 888-707-2500 to make an initial appointment for an adult at the Patient Admitting and Emergency Services Clinic. If you are calling for an emergency appointment, choose Option 1.

Call the Pediatric Dentistry Clinic at 734-764-1523 to make an initial appointment for a child under 14 years old.

Call the Dental Faculty Associates practice at 734-764-3155 to make a dental appointment with a member of the faculty.
What to Take

Take photo identification, either a driver's license or a state-issued ID card, to your first appointment.

Take your health-insurance card or other proof of dental/medical insurance.

Take a written referral from another dentist or physician and X-rays, if you have these documents.

Print the "Registration and Treatment Consent," "Health History," and "Consent" forms from the University of Michigan's School of Dentistry website.

Complete the forms you printed and take them to your first appointment.
Where to Go

Park in the Fletcher Street Parking Structure on the Plaza and Fifth levels.

Enter the School of Dentistry through the north entrance that faces the parking structure.

Ask for directions at the Information Desk on the first floor.

Use the automated directional system for the School of Dentistry, if you prefer. Call 734-763-6933 or 888-707-2500 and select Option 4.
