Determine your income eligibility. Eligible individuals or families cannot exceed $16,608 per year for a family of three or fewer. Families with disabled people or seniors may be eligible for food stamps if their gross income is $2,297 per month or less.

Gather required application information such as the Social Security numbers and birthdates of all household members. Also have asset information, housing and utility expenses, and all family members' income information available.

File an application. To apply online, go to the Utah Department of Workforce Services. Click the Food Stamps link under the Assistance heading, then click on Apply. Contact or visit your county's Utah Department of Workforce Services office to request a paper application.

Review and submit the online application. Create a username and password if you need to complete and submit the application at a later date. Write down your confirmation number once the online application has been submitted.

Expect your application to be reviewed within 30 days. The Utah Department of Workforce Services will mail you a letter with your scheduled interview date and time. To qualify for food stamps, you must attend the interview and meet with a caseworker to review your eligibility.

Attend your scheduled interview on time. Bring in required documents listed in your interview schedule letter, including valid identification, your Social Security card and proof of income.

Request another date or a phone interview if you cannot attend the in-office interview by contacting your county's Utah Department of Workforce Services.

Await an acceptance letter from the Department of Workforce Services approving your food stamp allotment amount, your Utah Horizon EBT card and PIN number. If you are denied benefits and would like to be reconsidered for eligibility, request an appeal in writing to your county's Utah Department of Workforce Services.
