Print and fill out the Combined Application Form on the Minnesota Department of Human Service Website (see Resources). Mail or drop off your application at your local Human Services Agency. This application is also available at any Human Service Agency in Minnesota.

Go to your county's Human Services Agency (see Resources) and submit the application for food support. You will be interviewed while you are at your local Human Services Agency. Bring with you proof of residence and identity, such as your driver's license or passport; proof of monthly income from your job, such as a pay stub or W2; and your monthly expenses, including mortgage or rental statements and utility bills. If you mail your application and cannot attend your interview, a caseworker will request a phone interview with you.

Wait for approval within 24 hours for emergency situations and no longer than 30 days for other applicants. Look for your food stamp card in the mail. The food stamp card is similar to a credit or debit card and is used like one at grocery stores. You will receive a monthly allowance to be used towards groceries. Allowances are determined by number of people in your household and your total income.
