-
2
Locate and click on the "Family Support" link on the Department of Social Services website. Click on the Office of Family Support's "Supplemental Nutrition Assistance Program (Food Stamps)" link under the headed section "Programs for Families." Access the "Application for Assistance" to download the SNAP application form.
-
3
Print out the SNAP food stamp application. Use pertinent information and documents such as your social security number, last four check stubs or unemployment worksheet and any current bank statements to complete the application.
-
4
Mail the application in to your local Office of Family Support Parish. Visit your local OFS Parish office to fill out and submit a paper SNAP application.
-
5
Wait for a Louisiana Department of Human Resource caseworker to send a letter confirming the status of your SNAP application. A mandatory interview with a date and time will be highlighted in the letter.
-
6
Attend the mandatory interview on time so that the caseworker can fully review your eligibility. Bring valid identification, your social security card, last four recent check stubs or self-employed income tax return, rent or mortgage receipts, a utility bill and recent medical receipts (if available) to the interview.
-
7
Request an optional date or a phone interview if you cannot attend the in-office interview.
-
8
Await an acceptance letter approving your SNAP benefit monthly amount, your Louisiana Purchase Card and PIN number from the Department of Human Services. Request an appeal in writing to your local Department of Human Services if you are denied food stamp benefits and wish to be reconsidered for eligibility.
