How to Find Shot Records for Texas Children

Children are required to have had certain vaccinations prior to attending public and most private schools, as well as when registering for certain extracurricular activities. Shot records may be obtained at the request of a parent or legal guardian by contacting the doctor or medical facility directly. When the doctor's office is unreachable, shot records for Texas children may be found by searching a citywide vaccine registry, if available, or the statewide immunization registry of Texas, known as ImmTrac.

      Contact the pediatrician or medical facility where the child received her vaccinations. Request a copy of the vaccines or shot records. Provide the child’s full legal name and date of birth when requesting shot records. Depending on the rules or regulations of the office, a parent or legal guardian may be required to visit the location in person or complete a form to request shot records.

      Contact the vaccine registry of the city in which the immunizations were given, if available. City registries, such as the San Antonio Immunization Registry System, contain immunization records for all registered children and may be requested by mail. Complete the Request for Immunization Record in its entirety, including the child's full legal name, child's date of birth, sex, mother's maiden name, mother's date of birth, relationship to the child, current address and phone number. Include payment for any required fee before mailing.

      Call ImmTrac, if the pediatrician is unavailable or indefinitely unreachable. ImmTrac is the Texas Immunization Registry provided by the Texas Department of State Health Services. It contains electronic shot records of Texas children and is accessible by health care providers, child care facilities, schools and public health providers. Requests must be made via the ImmTrac toll free number (800-252-9152) and verification of the child’s name, birth date, address and relationship to the child is required to access the information.
