How to Request a New Medicaid Card

Medicaid is a federally funded program, administered by each state, that provides medical coverage for individuals living at or below the federal poverty level. Persons eligible for Medicaid are supplied with a Medicaid identification card. This card includes the person's name, Medicaid ID number and photograph. A Medicaid recipient who has lost or damaged his card may request a new Medicaid card by calling or visiting his local department of social services.

      Contact your local department of social services or welfare department (where you went to apply for Medicaid and/or were interviewed to determine that you are eligible to receive Medicaid).

      The department of social services or welfare department may give you a temporary ID card to use until your permanent replacement card arrives in the mail, usually about 10 business days after you request a new card.

      Watch the mail for your new Medicaid card. If your new Medicaid card does not arrive within 10 business days, call the department of social services or welfare department.
