Oregon FHP: How to Apply for Family Health Plus
For adults whose income is too high to qualify for Medicaid, the largest source of funding dedicated to medical and health-related services for people in the United States who have limited income, finding affordable insurance can be challenging. Family Health Plus is just one name for a state-run public health insurance program available to adults aged 19 to 64 whose income precludes them from receiving Medicaid. In Oregon, such insurance is available through the Oregon Health Plan, or OHP. OHP Standard provides free or low-cost health-care coverage to Oregon residents who have limited income, are age 19 and older, and who do not qualify for traditional Medicaid.Instructions

Add your name to the OHP Standard Reservation List. Register online, or call 1-800-699-9075 to request a reservation. For TTY only, call 503-373-7800. If your name is selected from the list, you will receive an OHP Standard application.

Fill out the OHP Standard application in its entirety. Call 1-800-699-9075 if you need the application in Braille, large print, on an audio tape, on a computer disk, spoken over the phone or in a language other than English.

Make copies of documents that show the information you gave is correct. Include proof of identity; citizenship; pregnancy, if applicable; income from work and any unearned income.

Submit your application and copies of all supporting documents. They can be mailed to OHP Processing Center, PO Box 14520, Salem, OR 97309-5044, or faxed to 503-373-7493.
