  Locate a provider by clicking on the drop-down arrow for "Select Your Network," and selecting either "Access," "Select," "Advantage" or "Insight." Enter information for your street, city and select the state from the drop-down menu or enter your zip code.

  Enter the security text appearing at the bottom of the page and click on "Submit."

  View the results to locate a provider most convenient to you and call the provider's number to confirm that EyeMed Vision Care is still accepted.
