How to Apply for Hawk-i Health Insurance for Kids

Created as a result of the federal Balance Budget Act of 1997, the Healthy and Well Kids in Iowa plan, more commonly known as the hawk-i plan, provides free or low-cost healthcare for children of families that qualify. In addition to the income limits for the plan, your children must be under 19 years old, ineligible for Medicaid, not covered by another health insurance plan, and be U.S. citizens or legal resident aliens.

      Determine if your family meets the income requirements of the hawk-i plan. You can find two tables containing the income limits by family size for the free plan and a plan that costs $10 per child on the Healthy and Well Kids in Iowa website.

      Download the application from the Healthy and Well Kids in Iowa website or call (800) 257-8563 to have one mailed to you.

      Gather the necessary documents to send with your application. Photocopy pay stubs for the past 30 days for your spouse and yourself to prove that your family meets the income requirements of the plan. The Healthy and Well Kids in Iowa website has a list of documents you can send to prove your child's identity and citizenship status.

      Complete the application by providing the requested information about yourself, your family members and your income. Sign the last page of the application.

      Mail the application and photocopies of the required income, identity and citizenship documents to Healthy and Well Kids in Iowa. You can also fax the application and documents to either 877-457-7701 or 515-457-7701. The program will review your application and first determine if your children are eligible for Medicaid. Your children gain enrollment in the program if you meet the income requirements and your children are not eligible for Medicaid.
