A variety of health insurance options are available based on your family's needs. Managed care programs allow participants to receive physical health services from a selected primary care provider. The primary care provider determines what other services may be needed. Costs are relatively predictable with monthly premiums and predetermined copayments for specific services. Direct access plans provide more flexibility when choosing service providers than managed care. Services are covered through an annual deductible and predetermined or percentage-based copayments. A family dental discount plan is available through the Horizon Centurion Dental program for a base annual fee of $84.00.
