      Create a chart using a spreadsheet or word processing document that includes your name, birth date, and blood type.

      Add information about your drug and food allergies, family illnesses and chronic illnesses, if applicable. List any medications that you take along with their side effects and durations of use.

      Write down the name and contact information of your doctors, pharmacist, emergency caregiver and insurance companies.

      Date your last doctor's visits, test results and health screenings. Include a brief description of every procedure.

      Request a copy of any test or procedure reports following your medical visits. Scan the copy into a digital record, or store the file along with your other reports. Store copies of medical bills and insurance claims, as well.
