How to Enroll in the Chip Program in Beaumont, Texas

Gather your personal records. You will be required to submit copies of your last pay stub, tax returns or proof of any other sources of income along with your application. Proof of your child's citizenship status and any childcare expenses you pay are also required. Locate your child's Social Security card, birth certificate or passport, daycare receipts and related documents and place them in a folder.

Determine if you meet the basic requirements for the program. Your CHIP application must be approved in order to enroll in the program, but you should be aware of the basic age and income requirements before applying. Obtain a CHIP application. If you visit chipmedicaid.org you may print the application or complete it electronically. You may also obtain an application from the pharmacy of any of the three H-E-B grocery stores in Beaumont or call 1-877-543-7669 to apply.

Complete and submit your CHIP application. Fill out the entire application. Print clearly and do not leave any blanks. Sign the application and submit copies of your documents with it. You may fax the application and documents to 1-877-542-5951 or mail it to the address provided on the application.
