How to File a Complaint Against a Pharmacy
Instructions

1
Contact the pharmacy first to discuss your complaint. In some cases, the issue is an honest mistake and the pharmacy can remedy the situation. However, if you feel the pharmacy didn't handle the issue appropriately or that the issue is severe, submit a complaint to your state's board of pharmacy.
-
2
Fill out the online form available on your state's board of pharmacy website. This form makes lodging a complaint easier. You must provide the pharmacy name and address, name of the pharmacist and details of your complaint. The forms often ask for your contact information, prescription information and whether you have tried to remedy the problem yourself.
-
3
Write a complaint letter to your state's board of pharmacy. Include all relevant information in regard to your complaint; who you are complaining about, what happened and where it happened. The more information you provide, the easier it is for the board to investigate the issue.
-
4
Call your state's board of pharmacy. Some people are better able to express themselves verbally rather than in writing. If you feel it is easier to talk to someone, lodge your complaint over the phone if your state allows it. Some states require that the complaint be made in writing.
