How to Get Doctors to Fill Out FMLAs
The Family and Medical Leave Act (FMLA) protects an employee's job while he recovers from an illness or cares for a sick family member. The patient's primary physician must complete the FMLA Provider Certification forms in a timely fashion, usually within 15 days of the onset of injury or illness.Things You'll Need

Schedule a late doctor's appointment. Ask for the last appointment of the day, which gives you more time with the doctor. Take this opportunity to ask your doctor to complete your FMLA forms.

Fax your FMLA forms to your doctor's nurse. On the fax cover letter, include the fax number to the benefits office of your employer. In addition, specify a return date.

Drop off your FMLA forms at your doctor's office. Attach a letter of request asking the doctor to complete the forms as soon as possible. Include a return fax number and mailing address. Provide a stamped addressed envelope, if you would like the forms mailed.
