Make a list of five to ten things that could trigger your addiction, such as seeing people you used to drink with or even seeing a glass of beer on television.

Write down solutions to each of these triggers. The solutions depend on what works for you, but an example could be to look at a picture of yourself at your worst.

Write down at least five people you can call if you have the urge to relapse. These can include members of your support group, friends, family or anyone who can effectively talk you through a craving.

Write down at least five activities that could get your mind off your urges. Make sure the activities are things that are relaxing and that you really enjoy, such as listening to music or taking a bubble bath.

List what you plan on doing in case you do relapse, such as calling your therapist. This can help you learn to control the situation and change permanently.

Make copies of your relapse prevention plan and keep them available. According to Dual Recovery Anonymous, being prepared for a relapse is the best way to prevent one.
