How to File a Complaint On a Rehab Facility in California

Determine what type of facility you are filing a complaint against. Is it a Community Care Facility, or is it a Health Care Facility? This will determine who you need to contact.
Community Care Facilities include residential facilities, such as in-patient rehabilitation facilities.
Health Care Facilities include facilities that are licensed by the California Department of Health Care Services, such as hospitals and health centers.

Attempt to resolve minor complaints or concerns with the facility on your own. However, if your concerns are not addressed, or the facility is breaking the law, a complaint should be filed, and they should be reported to local law enforcement.

Call, send an email or write to the appropriate office and explain what your complaint is. You can choose to remain anonymous if you wish.
For Community Care Facilities, contact the Adult Care Program Office through their website at CDSS.ca.gov or by telephone at 916-657-2592.
For Health Care Facility complaints, contact the Licensing and Certification Division of the California Department of Public Health through their website at CDPH.ca.gov or by telephone at 916-558-1784.

Contact the local licensing agency, if you have questions or concerns between the time you filed the complaint and the time that they told you that they would give you the results of their investigation.
