How to Report Pharmacy Mistakes

A pharmacy mistake is a very big deal. Many people rely on pharmacies to supply them with prescriptions that they need to live. If a pharmacy error is made, it can be very harmful, and maybe even deadly in some cases. If you are the victim of a pharmacy mistake, it is important to report it so that action is taken to help prevent the same error, or one of a similar nature, from happening again to another person.

  Go to the Institute for Safe Medication Practices (ISMP) website at "https://www.ismp.org/" and click on the "report errors" tab.

  Write an email to the ISMP describing the pharmacy mistake. Include information such as your reaction to the drug, whether or not it was a pharmacy error, the patient outcome, the type of practice the pharmacy is located in, both the generic and brand names of the medicine, the dosage method, the way the error was discovered, and your recommendation for error prevention in the future.

  Upload any images that you might have of the medication, the pharmacy, or your reaction to the drug.

  Include your email address with your submission, in case the ISMP needs to contact you. You have the option to include your name in the appropriate space, but you are not required to.

  Click on the "submit" button to send your pharmacy mistake report to the ISMP.
