How to Obtain a Medicare Provider Number in California

The population is aging, and more people will qualify for Medicare in the future. Doctors, physical therapists, social workers and others who want to provide care to the aging population of California must have a Medicare provider number to file for compensation. Receiving a number is not difficult, as long as you meet the enrollment requirements and agree to Medicare's guidelines.

      Read the enrollment requirements found at http://files.medi-cal.ca.gov/pubsdoco/prov_enroll.asp to determine if you are eligible. Whether you are a social worker, doctor or optometrist, you must be licensed by the state of California to enroll. The other requirements vary by profession, so make sure you read the enrollment regulations carefully.

      Submit a Medi-Cal Provider Application to the Department of Health Care Services, Provider Enrollment Division, MS 4704, P.O. Box 997413, Sacramento, California, 95899-7413. Make sure you have a National Provider Identifier (NPI), because you will need it to finish the application.

      Submit a Medi-Cal Provider Agreement to the Department of Health Services. By signing this, you agree to the terms, conditions and compensation amounts for the services you provide.

      Submit a Medi-Cal Disclosure Statement. In this document you state how much of the medical facility you own, your earnings and whether you have been convicted of any crimes.

      Receive your Medicare Provider Number, once the California Department of Health Services approves your application.
