Will blue care network of Michigan cover a tubal reversal procedure?

Blue Cross Blue Shield of Michigan (BCBSM) does cover tubal reversal for members with Preferred Plans, or for members with POS/PPO/HDHP plans that have the maternity rider as part of their coverage.
