How to Check on My File Number With the VA Hospital
When you receive medical services from a Veterans Administration hospital, medical records will be generated. Part of these medical records is a file number. If you ever need to access your VA medical history in the future, you may need the file number to get to all of the information you need. With the file number, it might also be possible for someone else to obtain the medical records for you, depending on the specific policies of the VA hospital you were treated by.Instructions

Locate the VA hospital that treated you. The Department of Veterans Affairs offers a convenient search feature on their website which allows you to search for VA hospital by zip code. Select "Hospitals" in the "Facility" dropdown menu. Enter your ZIP code and click "Go."

Make a note of the address of the VA hospital that you were treated at. This information is provided by the Department of Veterans Affairs' website.

Write out your request for the file number associated with your medical records at the VA hospital. In most cases, only written requests will be honored. Include the dates of your treatments if known, your reason for wanting your file number and any other people that the information can be released to. Sign and date the written request.

Send the written request to the VA hospital or bring it to the hospital in person. Once received and processed, you should be provided with your VA hospital file number.
