How to Sign up for CMAPRX

CMAP is a program based in Louisiana for people that cannot afford chronic care prescription drugs. Louisiana residents with limited or fixed incomes are provided with medications to help treat chronic disease. CMAP also educates residents about the prevention and care of chronic disease. The program is strictly for Louisiana residents at or below the federal 200 percent poverty level and are without any type of health insurance.

Instructions

      Go to the CMAPRX website to sign up for the program. Click on the "Apply" link at the top of the page and click "Patients."

      Enter your name, address, date of birth, Social Security number, employer and income information. Answer all of the questions asked of you, such as whether or not you smoke.

      List all of the medications you are currently taking. Sign all of the remaining documents in the presence of a notary public. You must reenter your personal information on all of the pages where lines are provided for your name and Social Security number.

      Make a copy of your photo identification and send it, along with all signed and completed pages of the application to:
