How to Change Incorrect Information on Your Medical Records in Indiana
The Health Insurance Portability and Accountability Act of 1996 (HIPAA) and the state of Indiana, if you're a resident, provide you with the right to amend certain items in your medical record. This includes information that identifies you; information that you tell your doctor, examination results, test results, treatment received in a hospital; x rays, records made by heart monitors, medicine prescribed, as well as notes your doctor makes about you. Verify the Information You Can Change

Complete your health care provider's medical record access request form, or write a letter to the provider requesting access to the medical record. Review your medical record and check it to see if there is something that you would like to amend.

Complete a provider amendment request form to have the provider amend information or write another letter to the provider informing her that you want to amend your medical record. If you need to write a letter, make sure to include the date of your request, your name, and address. Include any other contact information, your date of birth, Social Security number; the type of information you want to amend; the information you believe is incorrect or omitted; the information you wanted added to the record, and the reason you want to amend your medical record.

Wait to hear from the requested provider the required 60 days to see if your request was denied or amended. If the provider decides to amend your request she must provide in writing what she is amending and she is also obligated to notify other providers affected by the amended record.

Give the provider contact information about the other providers or essential people who need to be notified that your record has been amended.

Write another letter or note that must be added to the medical record that expresses your disagreement with what is recorded in the medical record, if the request was denied. The provider must notify you in writing why it was denied.
