Call you state's Board of Nursing for specific requirements. Call your state's Board of Nursing, located in your state capital, and inquire as to its requirements for a replacement CNA nurse aide registry card. If you are able to request the replacement by mail and plan to do so, record the complete address of where to send the request.

Send a letter to your state's Board of Nursing ASAP. Write a letter requesting a replacement CNA nurse aide registry card to your state's Board of Nursing. Include your full name, address, telephone number, social security number and CNA registry number if you have it.

Purchase and include a money order for the replacement cost, if the state Board requires payment. Most state boards of nursing will not accept personal checks and require a cashier's check or a money order. A replacement license card cost ranges from free to $25, as of April 2011. Mail your letter and money order immediately.
