How to Get Your CNA Nurse Aide Registry Card If You Lost it
If you have lost your Certified Nursing Assistant (CNA) Nurse Aide Registry card, call your state's Board of Nursing to determine what the organization requires to provide a replacement. Some states allow you to complete the replacement request online. If you have this option and Internet access, ask for a review of this procedure and how to find the application online. Others allow you to print out a PDF form from their websites and mail them in. Still others accept a request by letter as long as you identify yourself adequately. States differ, too, as to charges for a replacement. Some states provide replacement cards at no cost, while others charge as much as $25 per duplicate license issued, as of April 2011.Instructions

Call your state's Board of Nursing, located in your state capital, and inquire as to its requirements for a replacement CNA nurse aide registry card. If you are able to request the replacement by mail and plan to do so, record the complete address of where to send the request.

Write a letter requesting a replacement CNA nurse aide registry card to your state's Board of Nursing. Include your full name, address, telephone number, social security number and CNA registry number if you have it.

Purchase and include a money order for the replacement cost, if the state Board requires payment. Most state boards of nursing will not accept personal checks and require a cashier's check or a money order. A replacement license card cost ranges from free to $25, as of April 2011. Mail your letter and money order immediately.
