How to Transfer an LPN From Oklahoma to Arkansas

Transferring your LPN license from Oklahoma to Arkansas will require that you formally apply to the Arkansas State Board of Nursing for an Arkansas LPN license. In order to avoid unemployment while waiting for your new state license, Arkansas will issue a temporary license for use while your formal license application is processed.



If both states were members of the National Licensure Compact agreement, the transfer would be expedited by the Compact's guarantee of license reciprocity. Unfortunately, Arkansas is a member while Oklahoma is not. This means that the application will require more documentation than expected.

• Proof of residency (limited to one of the following: driver's license, voter registration card or federal income tax return)
• Signed affidavit
• Notary public's services
• $25 by credit card, cashier's check or money order made out to ASBN for temporary Arkansas permit (optional)
• $100 by credit card, cashier's check or money order made out to ASBN for Arkansas LPN license
• $41.25 by cashier's check or money order made out to ASBN for criminal background checks
• FBI fingerprint card (ASBN-provided and professionally executed only)
• Fingerprint professional
• Copy of current Oklahoma LPN license
• Completed Arkansas LPN licensure application
• Completed Arkansas criminal background form
• Completed FBI fingerprint card
• Completed Arkansas Verification form
• Letter-sized envelope

  Your first step is to locate the application online.

  Go to the Internet website http://www.arsbn.arkansas.gov/licensing/Pages/endorsement.aspx, and click on the blue "Endorsement and Instructions - PDF" link.

  Arkansas requires state and federal criminal background checks on all applicants. The form is not downloadable and must be mailed to you. Click "Yes" to request the federal form via mail and to continue.

  Wait. A temporary screen will appear saying "thank you," advising you not to contact the Arkansas State Board of Nursing for at least two weeks and indicating that the required endorsement applications will begin loading momentarily.

  There will be three separate documents and instructions to print.

  Print out the documents when loaded. This will include detailed instructions, an Arkansas State Board of Nursing Endorsement Application, an Arkansas State Board of Nursing: Arkansas State Police Criminal Background Check Form and an Arkansas State Board of Nursing Verification Form.

  Locate the Arkansas State Board of Nursing Verification Form. Fill in your name, license number and address. Complete the form and mail it to:

  Locate the two-page Arkansas State Board of Nursing Endorsement Application. Fill in all required information on Page One and Page Two. Do NOT fill in or sign the Affidavit at the bottom of the second page.

  Locate the one-page Arkansas State Board of Nursing: Arkansas State Police Criminal Background Check form. Fill in all required information. Do NOT sign your name to the document or complete any information below the box in bold.

  You must wait for part of the application to arrive by mail.

  Set your completed Application and Criminal Background Check aside. Wait for delivery by mail of the Federal Criminal Background Check Form and the Arkansas State Board of Nursing FBI Fingerprint Card.

  On receipt via mail of the requested documents, fill in the required information on the Federal Criminal Background Check Form and the Arkansas State Board of Nursing FBI Fingerprint Card. Do NOT sign the Federal Criminal Background Check Form. You may sign the FBI Fingerprint Card.

  Do not sign the documents requiring a notary public before directed to so do.

  Visit a notary public and provide the required identification documents (limited to one of the following: driver's license, voter registration card or federal income tax return) and any charges for their service. Sign the Arkansas State Board of Nursing Endorsement Application affidavit, the State Police Criminal Background Check form and the Federal Criminal Background Check form on the direction of and in the presence of the notary public.

  Your fingerprint card must be completed by professionals.

  Visit your local police or sheriff's department to have your fingerprints taken professionally. There may be a fee involved. The application instructions state that the Arkansas State Police ID Bureau on Geyer Springs Road at I-30 in Little Rock will perform this service at no cost Monday through Friday, from 8:00 a.m. to 4:30 p.m.

  Collect your completed Arkansas State Board of Nursing Endorsement Application, State Police Criminal Background Check form, FBI fingerprint card,

  $25 by credit card, cashier's check or money order made out to ASBN,

  $100 by credit card, cashier's check or money order made out to ASBN for Arkansas LPN license, $41.25 by cashier's check or money order made out to ASBN for the criminal background checks and a copy of your current Oklahoma LPN license. Place in manila envelope and mail to:
