How to Obtain My EMR
Many physician's offices and hospitals use electronic medical records (EMR), which are stored on a computer server, as opposed to written medical records. Patient's and guardians are able to request a copy of their medical records or their children's medical records at any time, according to the Health Insurance Portability and Accountability Act of 1996 (HIPPA) Privacy and Security. Visit or call your physician's office to determine their policy on obtaining a copy of your medical records.

Submit a request in writing or fill out the provider's records request consent form. Pay the office for the records, if required. Some offices will require this if the records are only for your personal use, but will not if they are needed for a medical referral to another physician.

Allow the office the required time to print copies of your EMR. Each office will usually have a policy on the length of time needed to have these records available for you, usually anywhere from five to ten business days.

Pick up your medical records from the physician's office or request that they be faxed to you.
