How to Transfer My Medical Records
The Privacy Act of 1974 prohibits the distribution of personal information, including medical records, without written consent, as stated by the U.S. Department of Energy's website (id.doe.gov). However, instances arise when you must transfer medical records from one provider, such as a lab, hospital or physician, to another. When this occurs, a medical release -- available from your medical provider -- supplies the necessary information and permission required by law for the transfer of your medical records.Things You'll Need

Visit your physician's office or hospital requiring your previous medical records. Carry two forms of identification with you. Make sure one form -- issued by the government -- contains your photograph. Use a driver's license, government issued ID or passport.

Request a medical records or medical information release form. This form gives your previous medical provider the information necessary to release your medical records.

Complete the form by providing your name, address, Social Security number, date of birth and telephone number. Sign and date the release form.

Give the completed form to the provider requesting the medical records transfer. The office then faxes or mails the release to your previous provider. The provider then sends the records requested directly to the requesting provider via fax or postal mail.
