How to Change a Name in CGFNS

CGFNS International is a global non-profit organization for nurse migration of immigrants from other countries. CGFNS provides services and products to health care professionals. This organization provides health care, nurse licensures, nurse registrations, education, and other services. CGFNS ensures the quality and eligibility of health care professionals by meeting the immigration, practice and licensure requirements in the U.S. If you are married or legally changed your name, you can change your name with CGFNS; unfortunately, you cannot change your name online.

  Write a letter to CGFNS International regarding your name change. Explain why you need to change your name on your CGFNS application, such as if you changed your name through marriage or legally changed your name for personal reasons. Include your signature (new name) on the letter.

  Make photocopies of your legal name change document(s) or marriage certificate. Go to your local public library to make photocopies of your legal documents if you do not have access to a copier.

  Send the letter and photocopy of your document(s) to CGFNS International.
