How to Report Bad Doctors in Pennsylvania

If you suspect malpractice or have a bad experience with a medical professional such as a doctor in Pennsylvania, file a formal complaint to the State Board of Medicine.       Download and print out a copy of the State of Pennsylvania Occupational Licensee Complaint form from the state of Pennsylvania website.

      Write down the events leading to your complaint on a separate piece of paper. Read the report and transfer information to the form. Fill out all fields on the complaint form. Reread it and check your facts. Let it rest for a night and read it again the next day to ensure the report is accurate and factual. If you have documents, test results or additional proof, make copies and include them with the completed report.

      Write a cover letter to accompany the form and documentation. Give a brief explanation of who you are, why you are writing and how to contact you. Keep the letter short. The form should contain all the important information. Make copies of the letter and the completed form.

      Place the cover letter, complete form and documentation into an envelope with postage and address it to the Bureau of Professional and Occupational Affairs, P.O. Box 2649, Harrisburg, PA 17105.

      Send copies of the form, letter and documentation to your health insurance company in addition to the Bureau of Professional and Occupational Affairs. The insurance company may decide to pursue further action should it deem it necessary.
