How to File a Complaint With the Texas Board of Medical Examiners
The Texas Board of Medical Examiners was established in 1907. In 2005, the Texas Board of Medical Examiners was renamed the Texas Medical Board. The Texas Medical Board is responsible for overseeing and licensing certain health care professionals, such as physicians and physician's assistants, practicing in the state of Texas. Included in this responsibility is the investigation of complaints against its licensees. If it finds a complaint against a member to have merit, the Texas Medical Board may place a member on probation, or suspend or revoke the member's license.Instructions

Determine if your complaint is within the jurisdiction of the Texas Medical Board. The Texas Medical Board only reviews complaints against MDs, DOs, PAs, and LACs.

Request a complaint form. You can do this by calling the Complaint Hotline at 1-800-201-9353. You can also get a complaint form from the Texas Medical Board's website -- just select 'Complaint Information' from the 'Information for Consumers' tab on the left-hand side of the page. On the "Complaint Information" page, select 'Place a Complaint.' Scroll down to the bottom of the page. There you will find links to the complaint form in various formats. Print out the form in whichever format you wish.

Fill out the complaint form. Include all details relating to your complaint, such as dates and details of each appointment.

Submit your completed complaint form and copies of your medical records to the Texas Medical Board. You can mail the completed forms to: Texas Medical Board Investigations Department, MC-263, P.O. Box 2018, Austin, TX 78768-2018.
