How to Get My Immunization Records

Immunization records are a medical listing of all the vaccinations that you, or your child, has received and the dates that they were administered. You might need a copy to register for school, travel out of the country or even for a health-related job. Obtaining an immunization record is not a difficult task if you have basic information.

      Contact the last pediatrician that you had as a child, or in the case of a child, contact the current pediatrician and ask them what the policy is regarding the immunization records. Some doctors will charge for each extra copy of the immunization records. They might require notice to get the records, so make sure that you request them before they are needed.

      Contact the state health department. They often have a copy of immunization records, especially if your job involves the health field in any way. If you have been stationed in the military, you can call and request immunization records from there as well.

      Call the last school that you attended. They keep immunization records on file for each student registered. Request a copy of your records. The school can require that you pick up the records.
