How do I Find an FQHC UDS Number?

The U.S. Health Resources and Services Administration (HRSA) uses Uniform Data System (UDS) numbers to identify and track Federally Qualified Health Centers (FQHS) and other organizations that receive HRSA grant funding. HRSA assigns a unique UDS number when it gives money to an organization or clinic, then uses the number to track the grantee's performance and treatment statistics. There are three simple ways to find an FQHC UDS number.

  Contact your clinic's grants administrator. She will have to know your clinic's UDS number for funding and reporting purposes. Be sure to explain why you need the UDS number. Since HRSA uses it for a variety of tracking purposes, your grants administrator will want to know how plan to use the number.

  Visit non-HRSA websites. The Rural Assistance Center (www.raconline.org) provides a number of helpful links, among them the Centers for Medicare and Medicaid Services' directory of all FQHCs, which includes UDS numbers. The Rural Assistance Center also offers a link to the Bureau of Primary Health Care reporting service, which provides a directory of all clinic sites sorted by UDS number.

  Call HRSA. The agency offers a toll-free number (1-877-313-1823) you can call for help in finding your health center's UDS number. You can also contact your clinic's regional project officer at HRSA's Bureau of Primary Health Care. HRSA provides a list of regional contacts on its website.
